Create global menu for switching accounts

If a user goes through the trouble of creating multiple accounts, the ability to quickly switch between them is important.
One possibility is to add just the account's avatar in the top right; clicking it opens a dropdown that lists all other available accounts. If there's only one account configured, this avatar/menu shouldn't appear.

Implemented. The menu appears in the top right if you have multiple accounts. It should stay in sync with the Preferences > Accounts list, regardless of which method you use for switching.

So! A couple things on this:
first off, it's way way easier. I like it lots.
- it doesn't seem to work with my identi.ca account. switching
from the prefs panel does, though.
- I have to say that I don't think the placement is great. I love that Spaz is looking more like a standard OS X app now; this really messes with that look. I wonder how else we could place this… maybe go with the dropdown menu for the Prefs button, which has an accounts list and then an option to open the prefs panel? Of course, now I'm thinking you wanted to do that before, and couldn't because of the Spry tab widget stuff.

I fixed this on identi.ca accounts. the issue is that it doesn't like periods in the element attribute. I switched to using the account hash as an identifier, and we're good.
